      Responding to an incident where a worker fell off a sidewalk shed step injuring back. Inspection reveal no notification was provided by the responder to the department as require by code as worker was taken to hospital for treatment. Additionally there were no barriers provided and maintain for work and storage zone at exposure one. Summons issued for condition, notify department as require and provide and maintain barriers.
