      At the time of my inspection of a 6 story new building under construction currently topped off at roof level. Responding to a incident job site which occurred on o2-14-2025. DOB records reveal that notification was not given to the department in a timely fashion as required which created a hazardous unsafe condition at the site. ECB violation issued for non compliance.

      At the time of my inspection of a 6 story mew building with side walk shed and scaffold removal at exposure 4. There was an incident on 02-14-2025 while workers were removing the temporary construction equipment. A worker was injured in the dismantling operation and was taken to area hospital. As per rule DOB should have been notified immediately as required, As of 02-18-2025 departmental reveals that no notification was made to the department creating a hazardous and unsafe condition on site.
