| Concrete residue came of building and landed on staging area at grade level. Incidents occurred 1100 AM and no notification was immediately made to the department as required. Incident was reported at 200 PM. and not immediately as required by code. Summons issued for condition, comply with code and immediately notify department as required going forward. | |||||
