      In response to incident where metal rod fell while being hoisted during pipe scaffold installation I observed pickup truck windshield was damaged by metal rod that fell from canvas bag being use for operation and there were no safe means of ingress and egress provided onto sidewalk shed. No safety measures provided or maintain for operation, Stop all exterior work and all scaffolding operation. Provide and maintain safety measures for operation going forward. Request CSE for reinspection. 2555
